Dr. Sanjay K. Madan, MD
In 1991, Dr. Sanjay Madan graduated from Medical College Baroda. After completing his medical training in New York in 1996, Dr. Madan relocated to Florida with his family to practice medicine in the Tampa/Clearwater area.
Dr. Madan’s accomplishments include, but are not limited to:
- President of the Medical Staff at both Mease Countryside and Dunedin Hospitals
- Board Member of the Morton Plant Mease Foundation
- Achieved Past Chairman of Medicine
- Member of the Pinellas County Medical Society
- Member of the American Medical Association
- Board Certified in Internal Medicine
- Member and Fellow of the American Board of Diabetes
He is also recognized by the American Diabetes Association and National Committee of Quality Assurance of Quality Diabetic Care.
Dr. Tiffany E. Hashem, MD
In 2012, Dr. Tiffany Hashem graduated from Rutgers – Robert Wood Johnson Medical School. After completing her medical training in Delaware in 2015, Dr. Hashem moved to Clearwater, Florida to provide comprehensive medical services to the underserved population in an outpatient primary care office.
Dr. Hashem’s accomplishments include, but are not limited to:

- Member of the American Medical Association
- American Board of Family Medicine
